Who follows this notice

Uses and disclosures of PHI without your authorization
- Treatment — coordinating your care with other providers, pharmacies, and labs.
- Payment — billing and collecting for services rendered.
- Healthcare operations — quality assessment, training, accreditation, audits.
- As required by law, including public health reporting, abuse or neglect reporting, FDA reporting, and judicial orders.
- To avert a serious threat to health or safety.
Uses and disclosures that require your written authorization
- Most uses and disclosures of psychotherapy notes.
- Marketing and the sale of PHI.
You may revoke an authorization in writing at any time, except to the extent we have already acted on it.
Your rights
- Request a copy of your record (we may charge a reasonable, cost-based fee).
- Request an amendment to your record.
- Request a list of certain disclosures we have made.
- Request confidential communications by alternative means.
- Request restrictions on certain uses and disclosures.
- Be notified of a breach of unsecured PHI.
- Receive a paper copy of this Notice on request.
Complaints
You may file a complaint with us at info@vivantacare.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to this Notice
We may change this Notice at any time. The revised Notice will apply to all PHI we maintain. The current version is always available on this page.
