What telehealth is
Telehealth is the delivery of healthcare services using interactive electronic communications, including secure messaging, audio, video, and the exchange of health information. Care may be delivered synchronously (real time) or asynchronously (store-and-forward).
Benefits
- Convenient access to licensed clinicians from your home.
- Reduced travel time and exposure to communicable illnesses.
Risks and limitations
- The clinician cannot perform a hands-on physical examination, which may limit the evaluation.
- Technology failures (poor connection, outages) may delay or disrupt care.
- Information you provide may be incomplete or inaccurate; the clinician relies on it.
- In rare cases, electronic transmission of protected health information could be intercepted despite security measures.
Alternatives
You always have the right to seek in-person care from a clinician of your choice instead of telehealth.
Your rights
- You may withhold or withdraw consent to telehealth at any time without affecting your right to future care.
- You have the right to access your medical record.
- The clinician may decline to provide care via telehealth if it is not clinically appropriate.
Prescriptions
Whether to issue a prescription, and which medication to prescribe, is solely a clinical decision. The clinician may decline to prescribe any specific medication.
Emergencies
Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
Acknowledgment
By proceeding with intake on Vivanta Care, you acknowledge that you have read this consent, had an opportunity to ask questions, and voluntarily consent to receive care via telehealth.
